CADI Waiver
The Community Access for Disability Inclusion (CADI) Waiver is Minnesota’s primary Medicaid waiver for adults with physical disabilities who want to live in the community instead of a nursing facility. It covers customized living support, personal care assistance, supported home living, and other community-based services. Eligibility: Must meet nursing facility level of care, be enrolled in Medical Assistance, and be age 65 or under (some exceptions apply).

Elderly Waiver (EW)
The Elderly Waiver (EW) provides home and community-based services — including customized living support — for Minnesotans aged 65 and older who would otherwise require nursing facility care. It covers personal care, daily living assistance, meal support, and more. Eligibility: Must be 65+, meet nursing facility level of care, and be enrolled in Medical Assistance.
Brain Injury (BI) Waiver
The Brain Injury (BI) Waiver is designed for Minnesotans living with an acquired or traumatic brain injury who need community-based care and support. Sister Support’s team has experience working with individuals navigating the unique challenges of life after brain injury. Eligibility: Must have a diagnosed brain injury, meet level-of-care criteria, and be enrolled in Medical Assistance.

Medical Assistance (MA

Private Pay
Sister Support also accepts clients who prefer to pay privately, without going through a Medicaid waiver program. Our team will discuss rates, service options, and care plans directly with you or your family.
