Sister Support provides personalized customized living support services in Minnesota including personal care assistance, medication reminders, meal preparation, housekeeping, mobility support, 24/7 supervision, and community engagement activities. Every service is tailored to the individual through a detailed, person-centered care plan developed with the client, their family, and their care coordinator.
Getting started is simple. Contact Sister Support to schedule a free consultation. Our intake team will assess your needs, help identify the right waiver program or funding option (such as CADI, Elderly Waiver, or Brain Injury Waiver), and begin building your personalized care plan. You can call us, fill out our online contact form, or ask your case manager to submit a referral directly.
Sister Support was founded on a personal story of sisterhood and a belief that every person deserves care that feels like family. We prioritize consistent caregivers, person-first values, and a home-like residential environment. Unlike large institutional providers, we offer a warm, community-rooted approach to customized living support in Minnesota where relationships are at the heart of everything we do
Sister Support provides 24/7 supervision and support, conducts regular care plan reviews, and maintains open communication with families and healthcare providers. Our staff are trained in emergency response, medication management, and person-centered care practices — all within a safe, comfortable residential environment across Minnesota.
Sister Support accepts clients through the Community Access for Disability Inclusion (CADI) Waiver, Elderly Waiver (EW), Brain Injury (BI) Waiver, and Medical Assistance (MA). We also accept Private Pay clients. Not sure which program applies to you? Our care coordinators will walk you through the eligibility requirements and help you access the right customized living support services in Minnesota.
We begin with a thorough needs assessment conducted with you, your family, and your care coordinator. We consider your daily routine, personal goals, health requirements, cultural preferences, and long-term vision. From there, we develop a detailed individualized care plan that is reviewed and updated regularly to make sure it always reflects your current needs.
